Is the ReBuilder covered by Insurance?

Most private insurance plans cover the ReBuilder; however ReBuilder Medical does not bill insurance companies.  You will need a prescription from your physician and our receipt of purchase and file for reimbursement after purchase.  Information contained on this page is not intended to be interpreted as a guarantee of coverage.  Coverage amounts vary from company to company, and your plan may have restrictions on out-of-network purchases.  ReBuilder Medical may be considered “out-of-network” by your provider.

When contacting your insurance company it will be helpful to provide them with the following device information when inquiring about coverage:
  • ReBuilder Model 2407, CPT code E0730 (or ReBuilder Model 300, CPT code E0720)
  • Conductive sock, Left, CPT code E0731 LT
  • Conductive sock, Right, CPT code E0731 RT
  • Conductive glove, Left, CPT code E0731 LT
  • Conductive glove, Right, CPT code E0731 RT

Below is a sample prescription of the information the physician will need to have on the prescription.

 

The conductive gloves can be added for an additional cost.  If the gloves are desired for treating upper extremity neuropathy they would need to be listed on the prescription with their corresponding CPT code.
